It was a bad day at the dentist. After all of the probing,scraping, and scratching, here are the results of the exam.

Occlusal fissure of the upper right 1st molar
Substantial lingual pitting of the right upper 1st premolar
An extraction of the upper right 1st premolar may be necessary
Mesial facial abrasion of the right upper 2nd premolar
Cervical distal pitting present on the right upper canine
Left lower central incisor attrition is evident
Lingual surface pitting of the upper right lateral incisor
Lingual gingiva recession at left upper central incisor
Mesial fissure of the right lower lateral incisor
Cervical pitting of the left lower 1st premolar
